Who we are

This notice concerns yusifzade.com, operated for inquiry processing by Individual Entrepreneur Rauf Vugarovich Yusifzade (Russian: ИП Юсифзаде Рауф Вугарович), Russian taxpayer ID 771896536323, sole proprietor registration number 324774600669405.

Information we collect

Our consultation form requests a name, a phone number or an email address, and an optional question. The form relates to a rhinoplasty consultation. Forms also capture referral page URL and UTM campaign parameters. Please do not include detailed medical histories, photos or records in web forms.

We process a visitor IP address transiently for anti-spam and security purposes for up to approximately 10 minutes, without retaining it in the inquiry database. Server security logs may contain technical identifiers independently of this transient anti-spam operation; their retention is described below.

If you allow optional analytics, Yandex Metrica may collect website activity, browser/device identifiers, visited pages and the event and topic associated with a submitted inquiry. Session replay/Webvisor is disabled. Contact details and inquiry text are not passed to analytics. Google Search Console is used for webmaster search-performance reporting; it does not itself install a visitor analytics tag.

How information is used

To answer inquiries, arrange potential consultations and maintain relevant correspondence; prevent automated abuse and protect site security; assess general website performance when analytics is permitted. We do not use inquiry contact details for marketing mailings and do not sell personal information.

Consultation topics and certain page URLs can indicate interest in medical services. We treat these as potentially sensitive health-related information, and apply additional notices and restrictions where law requires. See our separate Consumer Health Data Privacy Policy.

Recipients and destinations

Inquiry form submissions are emailed to tntnkaka@yandex.ru (Yandex Mail) and docyusifzade@gmail.com (Gmail) and are not stored in a website inquiry database. The website and its email routing originate on infrastructure hosted in Russia (Reg.ru); Yandex Mail processes mail in its service environment; Gmail can process data in other countries. We cannot claim that all inquiry data remain solely in Russia.

We do not routinely forward inquiry data to unaffiliated medical clinics, advertising networks or a CRM. Authorized personnel handling inquiries can access the two mailboxes as needed. Yandex and Google act as email service providers under their applicable terms, not as independent marketing recipients authorized by us.

Yandex Metrica receives limited browsing/event data where analytics is enabled. The interactive Yandex Map loads only if the visitor clicks “Show map”; this action sends network requests and technical information to Yandex Maps. Editorial blog content is managed in a Supabase system hosted in Frankfurt; we do not put website inquiry records or patients’ data there.

Medical records sent after an inquiry

If you later voluntarily email medical photographs, records or descriptions of symptoms, they are handled in the resulting correspondence rather than via the website form. Email may be unsuitable for especially sensitive materials. We will provide a more suitable method and additional notice/consent when required before soliciting such materials. Do not email sensitive records until the method has been confirmed.

Children and age restriction

Website inquiry forms are intended only for people aged 18 or older. A parent or legal guardian aged 18 or older may submit an inquiry about a minor, providing only information reasonably needed at this initial stage. Minors must not submit inquiry forms themselves. A check box alone is not identity or age verification; if we learn that a minor has independently provided information, contact us to request review and deletion as appropriate.

Retention and security

Ordinary inquiries and associated correspondence are ordinarily retained for up to 12 months after the last communication, unless an applicable legal obligation or an ongoing patient relationship requires a different period. If a person becomes a patient, any additional medical documentation is subject to a separately defined clinical retention schedule.

Form anti-spam IP processing lasts up to 10 minutes. Proposed server log and backup retention are 30 days each, subject to technical verification and actual configuration. Analytics identifiers follow actual Yandex settings and consent choices. We use HTTPS, access restrictions and administrative controls, while recognizing that no email or internet transmission can be guaranteed perfectly secure.

Your privacy choices and requests

You can request information about personal data we hold, correction or deletion, or withdraw consent where applicable, by writing to either contact email above. We verify identity and respond according to applicable law; exceptions may apply. We provide mechanisms for eligible residents of individual US states when their privacy laws apply. We do not discriminate unlawfully for exercising privacy rights.

Some US laws provide appeal or additional rights; we will address an eligible rights request under the relevant state law. If we deny an appeal, we will explain any additional regulator-contact option that the applicable law requires.
