Consumer Health Data Privacy Policy
Separate health-data notice · yusifzade.com · Effective October 12, 2026
Entity and contact
The responsible website operator is Individual Entrepreneur Rauf Vugarovich Yusifzade, INN 771896536323, OGRNIP 324774600669405. Health-data and privacy requests: tntnkaka@yandex.ru; docyusifzade@gmail.com. This is a separate notice to address consumer health data where US state law applies.
Categories of consumer health data we may collect
From initial inquiries: the consultation the inquiry relates to (rhinoplasty), health-related details voluntarily entered into an optional message, and context such as the page URL that may reveal interest in a specific procedure. After initial contact, a patient or prospective patient might separately email health-related descriptions, photographs or medical documents. These later materials do not pass through the web forms.
We do not ask for medical files through initial forms, and discourage submission of sensitive details there. Pages visited and inquiry-submission events can sometimes reveal health-related interest; analytics runs only if you allow it.
Sources and purposes
Sources: data you submit through the website; later email correspondence you choose to send; potentially health-revealing website activity if you allow analytics. Purposes: reply to your request, discuss suitability and practical arrangements for consultation, maintain needed correspondence, comply with applicable obligations and protect site security. We do not use consumer health data for advertising, profiling, sale or unsolicited promotional messages.
Service providers and disclosures
Initial inquiries are routed to exactly two recipient mailboxes: Yandex Mail at tntnkaka@yandex.ru and Gmail at docyusifzade@gmail.com. These email services necessarily process communications as service providers under their respective terms. Where optional analytics is enabled, Yandex Metrica may process page-view and submission event data. Yandex Maps can process technical data only after the map is requested.
We do not routinely disclose inquiry content to unaffiliated clinics, advertisers or an external CRM. We may disclose data if legally required, subject to applicable protections. If a later clinical-care workflow requires disclosure to a new clinic or provider, we will first provide applicable notice and obtain any separate authorization or consent that is legally required.
This policy distinguishes routine vendor/service-provider processing from independent third-party sharing. We do not sell consumer health data.
How to exercise your health-data rights
Email either address above to request confirmation of processing, access, correction, deletion, or withdrawal of applicable consent. We may need to verify identity before acting and may retain information where lawfully required. For rights governed by Washington law, eligible consumers may appeal a refusal by replying to our decision with “Privacy appeal”; we will provide any applicable regulator complaint route if an appeal is unsuccessful. Nevada and other eligible state residents may exercise rights applicable under their respective laws.
Consent and additional processing
To the extent Washington or other applicable law requires it, we will obtain clear affirmative consent before collecting health information beyond what is necessary to provide a requested service. Consent to disclose/share consumer health data, where required, is separate from consent to collect it. A blanket acceptance of this policy alone is not authorization for new third-party disclosures or for sale. New purposes, categories or recipients require updates and consent where applicable.
